Keep positive allergy state when merging patients

Previously, the staying patient record had the allergy
state of the existing record regardless of whether
the *merged in* record had a positive has_allergy state
or not. Now we set the allergy state of the staying
patient to "does have allergies" if the patient being
merged and deleted was labelled as such, regardless of
whether the staying patient previously had has_allergy
set to true or not.
